Panchakarma is the most comprehensive detoxification and regeneration system in Ayurvedic medicine and one of the oldest medical cleansing procedures in the world. The term comes from Sanskrit: Pancha means five, and Karma means action or therapy. Panchakarma is therefore, literally translated, the doctrine of the five cleansing actions.
The system was described more than 5,000 years ago in the classical scriptures of Ayurveda, in particular the Charaka Samhita and the Ashtanga Hridayam, two of the most fundamental medical texts of Indian healing traditions. What was developed then has lost none of its effectiveness today, because it is founded on a core principle: illness does not arise suddenly. Ama refers in Ayurveda to undigested metabolic by-products that accumulate in the tissues, organs and channels (Srotas) of the body. Vamana - Therapeutic Emesis
Vamana is the classical cleansing therapy for Kapha disorders and describes a controlled, medically induced form of therapeutic vomiting. It may sound drastic, but when performed correctly, it is a gentle and highly effective method for expelling toxins from the upper digestive tract, the lungs and the bronchial system.
The body is prepared over several days through oil massages and steam treatments. On the treatment day, the patient receives a specific combination of herbal drinks that induce the emetic response in a controlled manner. The expulsion occurs in several waves and is carefully monitored by the doctor throughout.

Virechana - Purgation Therapy
Virechana is the Ayurvedic cleansing therapy for the middle digestive tract and is considered the most effective procedure for detoxifying the liver, gallbladder and small intestine. It is the most frequently used Panchakarma therapy and is suitable for most patients.
Through the administration of specific herbal preparations, commonly based on Trivrit, Senna or Cassia - a complete intestinal emptying is induced, which loosens and expels toxic deposits from the liver, gallbladder and small intestine. Virechana always follows a multi-day preparation phase involving Snehapana (internal oil intake) and Swedana (sweat therapies).

Basti - Intestinal Therapy with Herbal Oils and Decoctions
Basti is considered in classical Ayurveda to be the most important of all Panchakarma therapies. The term describes medicated enemas, though in a form quite different from what most Westerners are familiar with. There are two main types: Anuvasana Basti (with medicated oils) and Astha Pana Basti or Niruhana Basti (with herbal decoctions, honey, oil and paste).
Basti acts directly on the large intestine, which in Ayurveda is considered the primary seat of Vata. Vata disorders, including anxiety, insomnia, joint problems, dry skin and chronic pain, respond particularly well to Basti. A typical Basti series consists of between 8 and 30 individual sessions, alternating between oil and herbal Basti treatments.

Nasya - Nasal Treatment and Cleansing
Nasya uses the nose as a gateway to the head, as the classical Ayurvedic principle states: Naso hi shiro dvaaram, the nose is the door to the head. Nasya therapy involves the introduction of medicated oils, herbal powders or juices through the nasal passages to cleanse and strengthen the head, brain, sense organs and upper respiratory tract.
Before the actual Nasya treatment, the face, neck and shoulders are massaged with warm oils and the tissues are opened with steam. Afterwards, a few drops of the medicated oil are administered into each nostril, accompanied by breathing exercises. The treatment lasts 20 to 30 minutes and frequently produces an immediate sensation of clarity, lightness and heightened perception.

Raktamokshana - Blood Purification
Raktamokshana is the fifth and, in modern Panchakarma practice, the least frequently used therapy. Classically it describes the expulsion of impure blood through various methods, including the use of medicinal leeches (Jalaukavacharana), cupping (Siravyadha) or bloodletting. In contemporary application, leeches and cupping therapies are the primary methods used.
Raktamokshana is particularly effective for conditions caused by Pitta excess in the blood: skin diseases such as psoriasis and eczema, as well as gout, rheumatism and certain liver conditions. The therapy is carried out exclusively following careful medical diagnosis and strict indication criteria.

Phase 1
Purvakarma - The Preparation
Purvakarma literally means the preparatory measures and lasts between 3 and 7 days depending on the programme. The goal of this phase is to loosen the toxins (Ama) embedded in the body's tissues, liquefy them and guide them towards the digestive tract, from where they can be expelled during the main phase.
The most important Purvakarma measures are:
Snehapana - Internal oil intake:the patient drinks a daily increasing amount of clarified butter (Ghee) or sesame oil with herbal extracts. This saturates the tissues with fat and loosens fat-soluble toxins from the deep tissue.
Abhyanga - Daily full-body massage with warm herbal oils to activate the skin and lymphatic system
Swedana - Sweat treatments in a steam cabin or through hot herbal baths to open the pores and bring toxins to the surface
Dietary preparation - Light, easily digestible food (usually Kitchari made from rice and mung beans), no alcohol, early bedtimes
Phase 2
Pradhanakarma - The Main Treatment
Pradhanakarma is the actual cleansing phase and the heart of the Panchakarma cure. Here the five classical therapies, or an individually selected combination of them, are carried out in a defined sequence. The sequence is not arbitrary: it follows the anatomical logic of the body, from top to bottom, from inside to outside.
During this phase, the patient rests a great deal, avoids exertion, screens and emotional stimulation. The body is in a state of heightened sensitivity, the nervous system is open, the cleansing processes are running at full intensity. Your doctor accompanies you daily, adjusts the intensity of the therapies and monitors your progress.
Accompanying therapies in this phase include: Kizhi (heated herbal pouch treatments), Shirodhara (forehead oil stream), Pizhichil (royal oil bath), Netradhara (eye treatment) and specific herbal preparations taken internally.
Phase 3
Paschatkarma - The Aftercare
Paschatkarma is the most underestimated phase of the Panchakarma cure, and at the same time one of the most important. After deep cleansing, the body is in a state of heightened receptivity. The digestive fire (Agni) is weakened, the immune system is regenerating, the tissues are still permeable.
In this phase, the body is gently rebuilt. The diet is gradually increased from light convalescent food towards normal Ayurvedic nutrition. Specific herbal tonics (Rasayana) strengthen the regenerated tissues and support the long-term effect of the cure. Yoga, Pranayama and meditation accompany the return to everyday life.

A complete, medically effective Panchakarma cure lasts a minimum of 14 days, the classical duration is 21 days, and extended programmes can span 28 or more days. Anything under 14 days may allow for preparatory and accompanying therapies, but not a full Panchakarma main treatment. The duration is always determined together with your doctor and depends on your constitution, your complaints and your goals.
No, Panchakarma is not a painful treatment. Most patients experience the therapies as intense and sometimes unfamiliar, but not unpleasant. During the preparation phase, drinking Ghee daily (Snehapana) can feel heavy on the stomach. During the main treatment, individual therapies such as Basti or Nasya may require a brief moment of adjustment, but are well tolerated. The accompanying massages and oil treatments are experienced by almost all guests as deeply relaxing.
Panchakarma is an intensive cleansing cure, and as with any deep detoxification, temporary accompanying reactions may occur. In the first few days, patients commonly report fatigue, mild dizziness, headaches or emotional sensitivity, these are not side effects in a negative sense, but signs that the detoxification process is working.
